Ascension Columbia St. Mary’s (CSM) welcomes you and your student group. We are happy to have you here as guests at our facilities. Below is information that will be helpful to you prior to bringing your students here.

The information on this page is intended to provide the basic information, forms and web-based training needed to study, work and learn safely atAscension Columbia St. Mary's (CSM). Prior to beginning your instructor experience at CSM, each instructor must complete each of the steps as outlined below.


Instructor Requirements

Step 1: Forms

Please complete the following forms and submit to your school coordinator.


Step 2: Mandatory Orientation Courses

Please review the following courses. Note that your progress is not tracked in each course – if you leave a course in the middle, you will need to start again from the beginning. Be sure to leave adequate time to complete each course. Upon completion of the courses, be sure to complete the Attestation Form.

Step 3: Electronic Health Records (EHR) and Other Clinical Training

All clinical students and others, as applicable, are required to complete the Electronic Health Record (EHR) and other web-based training modules found below.

(Students will be granted access to the EHR once their information is entered into the student data base by the school coordinator and compliance forms are received. Usernames and Passwords will be sent to the Preceptor or Instructor.)

Please Note: Confirmation of orientation needs to be completed once per calendar year (January-December). If you are starting a second clinical rotation since the beginning of the calendar year, you do not need to complete the confirmation. If you try to confirm completion for a second time, the database will recognize your name and state that confirmation has already been completed.
